Each side pictures is a suggestion for you. What I have pictured is:
Top - Special Note
Front - First initial of Name
Right -Names of Parents
Back - Date of birth, Time, Weight, and Length
Left - Baby First, Middle, and Last Name
Bottom - Day of the week along with foot prints
This cube measures 2" x 2" x 2". It is made from hard maple and left unfinished. It is sanded smooth to remove any sharp edges. 
There are six sides that can be custom engraved and personalized just for you!
Any formatting for date, weight, and length will be the way I engrave the block. Please check spelling of everything.
